Dean’s/College Certificate Forms

Dean’s or College Certification Forms are required as part of the application for some law schools. This form must be filled out by a dean or other sanctioned official in your college. This individual must fill out the form regardless of whether or not they have personal knowledge of you as a candidate. Not all law schools require this form as part of their application.
Students or Alumni of LAS:

In the College of LAS, the certification forms are processed by the pre-law advisor. If you are a current student you can simply put the form in an envelope and bring it to the LAS Advising Center and ask that it be put in the pre-law advisor’s mailbox. Please be sure to include your UIN. If you are no longer on campus, please send the forms, along with a note that has your UIN and your graduation year/term on it, to:

Forms may also be submitted electronically to lasprelaw@uic.edu. You must include your full name, UIN, and graduation term and year in your email.

Students or Alumni of Other Colleges:

Your form must be filled out by an official from your college. Please contact your UIC college office to find out to whom the forms should be directed. The LAS pre-law advisor does not have access to the records of non-LAS students and, therefore, is not the correct person to whom to submit these forms.

All Students:

Please keep in mind that we all have multiple obligations; therefore, if you drop off the form the day before a deadline, please do not expect it to be processed by that deadline. Dean’s forms may take 1 to 2 weeks to get processed. Please plan accordingly!
